Disability registration will provide a more accurate picture of the distribution of persons with disabilities in the country. Subsequently, it will help the government and relevant agencies to plan policies, support programmes, services, education and training, etc.<\/span><\/p><p><span style=\"color: #000000;\">Persons with disabilities holding an OKU Card will have access to a range of\u00a0<a style=\"color: #000000;\" href=\"https:\/\/docs.google.com\/document\/d\/1hlqc3T2B4uTae_ln56aXyTxRGzQ21Ou6\/edit?usp=sharing&amp;ouid=118297902883256093713&amp;rtpof=true&amp;sd=true\">benefits and privileges<\/a>\u00a0(social protection programmes) provided by various government agencies for persons with disabilities.<\/span><\/p><p dir=\"ltr\">\u00a0<\/p><h2><span style=\"color: #000000;\">How Do I Register for an OKU Card?<\/span><\/h2><p><span style=\"color: #000000;\">You may register for an OKU Card online with the Department of Social Welfare (Jabatan Kebajikan Masyarakat, JKM) by:<\/span><\/p><ul><li><span style=\"color: #000000;\">Filling in the online\u00a0<a style=\"color: #000000;\" href=\"https:\/\/oku.jkm.gov.my\/daftar_oku\/create\">MyDaftarOKU<\/a>\u00a0form, or<\/span><\/li><li><span style=\"color: #000000;\">Fill up the\u00a0<a style=\"color: #000000;\" href=\"http:\/\/chrome-extension\/\/efaidnbmnnnibpcajpcglclefindmkaj\/https:\/\/oku.jkm.gov.my\/storage\/document\/download\/Borang_Pendaftaran_OKU_-_BPOKU_Pindaan_2019.pdf\">Borang Pendaftaran OKU Form<\/a>\u00a0and visit your\u00a0<a style=\"color: #000000;\" href=\"https:\/\/www.jkm.gov.my\/jkm\/index.php?r=portal\/locations&amp;map_type=05&amp;id=VDV6WnF6OVBRbWNNOGJYY1Rnd0pvdz09\">state JKM office<\/a>\u00a0in person.<\/span><\/li><\/ul><p><span style=\"color: #000000;\">You may obtain the OKU registration form from:<\/span><\/p><ol><li><span style=\"color: #000000;\">State and district JKM offices<\/span><\/li><li><span style=\"color: #000000;\">Government clinics\/hospitals<\/span><\/li><li><span style=\"color: #000000;\">State and district Education Department offices (Special Education Department)<\/span><\/li><li><span style=\"color: #000000;\">Download\u00a0<a style=\"color: #000000;\" href=\"https:\/\/oku.jkm.gov.my\/storage\/document\/download\/Borang_Pendaftaran_OKU_-_BPOKU_Pindaan_2019.pdf\">Borang Pendaftaran OKU<\/a>\u00a0from JKM\u2019s website<\/span><\/li><\/ol><p dir=\"ltr\">\u00a0<\/p><h2 dir=\"ltr\"><span style=\"color: #000000;\">The Purpose of Registration<\/span><\/h2><p dir=\"ltr\"><span style=\"color: #000000;\">The registration of persons with disabilities (OKU) is required in order to:\u00a0<\/span><\/p><ul><li dir=\"ltr\"><p dir=\"ltr\"><span style=\"color: #000000;\">Ascertain the population, distribution, and categories of disabilities in the country in order to better plan prevention, education, habilitative, and rehabilitative programs, training, and services;<\/span><\/p><\/li><li dir=\"ltr\"><p dir=\"ltr\"><span style=\"color: #000000;\">Plan and provide suitable facilities that cater to the needs of persons with disabilities (OKU); and\u00a0<\/span><\/p><\/li><li dir=\"ltr\"><p dir=\"ltr\"><span style=\"color: #000000;\">Enable registered persons with disabilities (OKU) to obtain services that cater to the needs of their specific level of functionality \/ disability.<\/span><\/p><\/li><\/ul><p dir=\"ltr\">\u00a0<\/p><h2 dir=\"ltr\"><span style=\"color: #000000;\">New Registration<\/span><\/h2><p dir=\"ltr\"><span style=\"color: #000000;\">New registrations are for those registering as a person with a disability (OKU) for the very first time. The following documents are required when applying for registration:<\/span><\/p><p dir=\"ltr\"><br \/><span style=\"color: #000000;\">i. A Persons with Disabilities Registration Form (BPPOKU-Pindaan 2019) signed and stamped by a Medical Officer \/ Medical Practitioner who is registered with the Malaysian Medical Council (MMC) only or a Medical Specialist who is registered with the National Specialist Register of Malaysia (NSR) only who is currently practicing at a government or private hospital or clinic;<\/span><br \/><span style=\"color: #000000;\">\u00a0ii. One (1) copy of the applicant\u2019s birth certificate \/ MyKid \/ MyKad;<\/span><br \/><span style=\"color: #000000;\">iii. One (1) passport-sized photo for the applicant\u2019s bearing a birth certificate \/ MyKid.<\/span><br \/><span style=\"color: #000000;\">\u00a0<\/span><\/p><h2 dir=\"ltr\"><span style=\"color: #000000;\">Re-Registration<\/span><\/h2><p dir=\"ltr\"><span style=\"color: #000000;\">Re-registrations are for persons with disabilities (OKU) bearing an old OKU Card which was registered before 2011. Applicants are required to fill-in the Persons with Disabilities Registration Form (BPPOKU-Pindaan 2019).<\/span><br \/><span style=\"color: #000000;\">Only the following cases require verification by a Medical Officer \/ Medical Practitioner \/ Medical Specialists:<\/span><\/p><ul><li dir=\"ltr\"><p dir=\"ltr\"><span style=\"color: #000000;\">Any and all categories of persons with disabilities (OKU) who do not have a sub-category listed in any written reports. This information is required in order to update your sub-category in the Information Management System for Persons with Disabilities (SMOKU);<\/span><\/p><\/li><li dir=\"ltr\"><p dir=\"ltr\"><span style=\"color: #000000;\">Persons with physical disabilities that are not clear or obvious.<\/span><\/p><\/li><\/ul><p dir=\"ltr\">\u00a0<\/p><h2 dir=\"ltr\"><span style=\"color: #000000;\">Card Replacement<\/span><\/h2><p dir=\"ltr\"><span style=\"color: #000000;\">An application for a replacement OKU Card may be submitted for the following reasons:<\/span><\/p><ul><li dir=\"ltr\"><p dir=\"ltr\"><span style=\"color: #000000;\">Damaged \/ Missing OKU Card.<\/span><\/p><\/li><li dir=\"ltr\"><p dir=\"ltr\"><span style=\"color: #000000;\">Change of address. Applicants are required to change the address on their MyKad first before submitting an application for change of address on their OKU card, and<\/span><\/p><\/li><li dir=\"ltr\"><p dir=\"ltr\"><span style=\"color: #000000;\">Listing a sub-category of learning disability; as listed in the sub-category of the Information Management System for Persons with Disabilities (SMOKU) or any written report; on the OKU Card.<\/span><\/p><\/li><\/ul><p dir=\"ltr\"><span style=\"color: #000000;\">Card replacements do not require the verification of a Medical Officer \/ Medical Practitioner \/ Medical Specialists.<\/span><\/p><p dir=\"ltr\">\u00a0<\/p><h2 dir=\"ltr\"><span style=\"color: #000000;\">Temporary Identification Document For Persons with Disability (OKU)<\/span><\/h2><p dir=\"ltr\"><span style=\"color: #000000;\">A Temporary Identification Document for Persons with Disabilities (OKU) is issued upon approval of your registration application. It is only issued at the request of the applicant \/ next of kin and is valid for thirty (30) days while the OKU Card is being prepared.\u00a0<\/span><\/p><p dir=\"ltr\">\u00a0<\/p><h2 dir=\"ltr\"><span style=\"color: #000000;\">Appeal<\/span><\/h2><p dir=\"ltr\"><span style=\"color: #000000;\">Applicants may appeal a rejected registration application by writing a formal request to the \u00a0 Chief Registrar. This along with the applicant\u2019s social report is then to be submitted to the OKU Registration Committee under the National Council for Persons with Disabilities on behalf of the Minister for deliberation.\u00a0<\/span><\/p><p dir=\"ltr\">\u00a0<\/p><h2 dir=\"ltr\"><span style=\"color: #000000;\">Cancellation of OKU Registration<\/span><\/h2><p dir=\"ltr\"><span style=\"color: #000000;\">1. OKU registration is canceled when the OKU Cardholder;<\/span><\/p><ul><li dir=\"ltr\"><p dir=\"ltr\"><span style=\"color: #000000;\">has passed away; or<\/span><\/p><\/li><li dir=\"ltr\"><p dir=\"ltr\"><span style=\"color: #000000;\">recovered to the extent that the disability no longer affects his\/her functionality.<\/span><\/p><\/li><\/ul><p dir=\"ltr\"><span style=\"color: #000000;\">2. \u00a0The OKU cardholder \/ next of kin is required to return the OKU card at the nearest District Social Welfare Office.\u00a0<\/span><br \/><span style=\"color: #000000;\">\u00a0<\/span><\/p><hr \/><p>\u00a0<\/p><p><span style=\"color: #000000;\">If you have questions or need further clarification, please contact your state JKM Office (<span style=\"color: #ccffcc;\"><a style=\"color: #ccffcc;\" href=\"https:\/\/oku.jkm.gov.my\/hubungi_kami\"><span style=\"color: #008000;\">JKM State Offices Contact<\/span><\/a><\/span>).
